A record of questions, not a medical assessment

Use this checklist alongside the actual quote and purchase agreement. A checked box means you have documented an answer, not that a product is right for you or that a benefit is approved. Ask the seller for written explanations and keep the originals. You can use it before an appointment, during a phone call with your insurer, or when comparing two different service packages.

Work through an example

Suppose a quote says “hearing system” without specifying the number of devices. Leave the device-count item unchecked until the seller confirms whether the quoted amount is for one ear or both. If follow-up appointments are described as included, ask for the length of that inclusion and whether adjustments, cleaning, and replacement parts have different terms. No sample cost here is a real offer.

Storage and limits

Progress is stored locally in this browser. It is not shared across devices or sent to a server. Use Reset to clear this checklist; browser site-data controls can also remove it. Do not put health information into a shared browser record. The list does not test hearing, verify licensure, confirm coverage, or replace medical advice. For sudden hearing loss seek prompt medical attention rather than completing a shopping checklist.
